The Life Plan tool serves as a guide to community providers to engage youth in one-on-one or group interactions and tracking their progress towards making positive changes in their lives. Life Plan is designed to give providers the information and skills they need to help youth in the process of “Life Planning.” Determine in what area of their lives they would like to make a change, set goals and monitor their progress. Life Plans is for any young person ages 18-24 to set goals in the topic of education, work, self-care and sexual health.
